These three consecutive checklist pages (6 sides) were all flown to the Lunar Surface on Apollo 17, their purpose was as a reference for the Astronauts to configure the Lunar Module's Navigation and Guidance Systems
Gene Cernan has signed and inscribed page 1-21 as 'Flown To The Lunar Surface - Gene Cernan'.
Each page measures 8" x 5.5" and are stored in Acid Free sleeves which in turn are presented in a delux black binder with an embossed Apollo 17 Emblem.
